The Patient Care Tech (PCT) provides direct patient care under the guidance of the Registered Nurse or designated health care professional in accordance with federal, state, and local regulations and within the policies, procedures, and guidelines of the organization. Essential Job Functions include providing basic patient care such as care and comfort, vital sign measurement, personal care and hygiene, and assisting with mobility including unit-based specialty duties. Acts as liaison between patient and nurse to report changes and/or concerns. Provides high-level customer service to all patients, patient's family, visitors, and employees. Provides accurate, precise, timely documentation when applicable for patient care. Participates in hand-off/report of pertinent information regarding patient and departmental needs upon arrival and when leaving. Performs delegated tasks once competency has been validated. Mobilizes patients using therapeutic transfer techniques which foster independence and safety. Fosters independence by encouraging patients to complete tasks as independently as possible. Individualizes care based on patient's interests and hobbies. Licensing/Certification requires BLS Basic Life Support (preferred upon hire, must obtain from approved source prior to direct patient care) OR 1 year of relevant experience in a clinical setting. Must have completed one of the following: Successful completion of nursing aide training program OR Successful passing of STNA State Tested Nurse Aide (required if OH LTC), OR CNA Certified Nurse Aide, OR Nurses Aide state testing or completed a nursing program that had one clinical nursing course and one clinical rotation. Education: None required. Work Experience: Recent experience in acute care, physician's office, home health/hospice, or long term care facility preferred. Training: EPIC Electronic Health Record (EHR) training preferred. Skills include ability to communicate clearly, understand and follow directions, knowledge of patient and healthcare safety standards, infection prevention, clerical duties, teamwork, patient data collection and documentation, basic hygiene and activities of daily living care, feeding, hydration, nutrition assistance, bowel and bladder elimination assistance, ambulation and patient mobility assistance, use of clinical technology, compassionate care approach, accountability, and escalation of concerns. Working conditions may include periods of high stress, exposure to physical altercations, verbal abuse, use of physical restraints, exposure to high noise levels, bright lights, hazardous substances, body fluids, human blood, and infectious materials, constant interruptions, and prolonged periods of working alone. Patient population includes adults (18-64 years) and geriatrics (65 years and older).
